La Fitness Personal Training Contract

Welcome to La Fitness! Thank you for choosing our personal training services. In order to ensure that we are able to provide you with the best possible experience, we ask that you please review and sign the following contract.

1. Personal Training Sessions

1.1. Personal training sessions will be one hour in length.

1.2. Sessions will be scheduled at mutually agreed upon times and locations.

1.3. In the event that either party is unable to attend a session, the session may be rescheduled at the mutual convenience of both parties.

1.4. If a session is cancelled with less than 24 hours notice, the session will be considered forfeited and no refund will be given.

2. Payment

2.1. Payment for personal training services is due in full at the time of each session.

2.2. Payment may be made in the form of cash, check, or credit card.

2.3. There is a $25 fee for returned checks.

2.4. Credit card payments will be subject to a 3% processing fee.

3. Cancellation Policy

3.1. If you need to cancel a session, please do so with at least 24 hours notice.

3.2. If you cancel a session with less than 24 hours notice, the session will be considered forfeited and no refund will be given.

4. Refund Policy

4.1. There are no refunds for personal training sessions that are cancelled with less than 24 hours notice.

4.2. If you are not completely satisfied with your personal training experience, we will refund the unused portion of your package.

5. Liability Waiver

5.1. In consideration of being allowed to participate in personal training sessions, I hereby waive, release, and discharge La Fitness, its officers, employees, agents, and successors from any and all claims, demands, damages, rights of action, or causes of action, whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of La Fitness or its officers, employees, agents, or successors, or otherwise.

5.2. I understand that personal training is a physical activity that carries with it certain inherent risks. I agree to assume all risks associated with my participation in personal training, including but not limited to, the risk of injury or death.

5.3. By signing this contract, I acknowledge that I have read, understood, and agree to the terms and conditions set forth herein.
